Proof of Residency


At least one parent/guardian from all district 95 families must prove residency each school year.

The Residency Event will be held at Lake Zurich High School.

Monday, August 7th – 1:00 p.m. to 8:00 p.m.

Tuesday, August 8th – 1:00 p.m. to 8:00 p.m.

Makeup Date:  Tuesday, August 15th - 3:00 p.m. to 7:00 p.m.


REMINDER... High School students need to attend the Residency Event. A parent must have proven Residency at the Residency Event in order for their high school student to have their picture ID taken and receive their schedule.



The mandatory meningococcal vaccine (MCV4) for incoming 12th graders will be offered at the Residency Event at the health station. This vaccine is being administered by Osco Pharmacy.

Vaccine Information from Osco Pharmacy

Most PPO plans will cover the vaccine at no charge; however, HMO plans and Medicare will NOT pay for vaccines through a pharmacy based team (only through a doctor’s office). All pharmacy benefit and medical benefit insurance information must be provided at the time of vaccination. If copays exist, that payment is expected at the time of vaccination as cash, credit, or check to the Osco pharmacy team.  If no insurance coverage, then the full price of the vaccine can be paid in cash, check, or credit directly to the Osco Pharmacy team on site.
A signed, parental consent form is required for any student to receive this vaccine, but it is not necessary for the parent to be with the student.  The form can be found below:


Proof of Residency

  • After proving residency each family receives an envelope with the district calendar, a one page update from the district, elementary teacher assignments, middle school schedules, a postcard for each high school student (needed for the High School station to get student ID and schedule)

Food Service

  • Sign up or manage PushCoin account
  • Add money to lunch account
  • Speak to Sodexo (food service) representatives
  • Pick up a menu


  • Pay fees - cash, check, or credit card
  • Complete free/reduced lunch application
  • Purchase and pick up PE uniforms (middle and high school)
  • Pick up parking permit (for selected HS students only)


  • Review bus route
  • Speak to Transportation staff
  • Pick up Kindergarten name tags for bus

Health Services

  • Drop off physicals and other completed medical forms
  • Pick up additional health forms as needed
  • Speak to health office personnel
  • Pick up forms for third party student accident insurance if needed
  • Optional service: required 12th grade meningococcal vaccine (MCV4) - offered through OSCO Pharmacy

High School Only

  • VIP photos (freshmen, sophomores, and juniors) and pick up Student IDs
  • Pick up schedules
  • Fill out off-campus permission forms if appropriate
  • Pick up PE locks
  • Athletics – questions, participation packets, etc.


Three documents are required to verify residency. You must present proof of residency within Lake Zurich Community Unit School District 95 by providing one document from Category I AND two documents from Category II.

CATEGORY I (ONE document required)

  • Most recent property tax bill
  • Current mortgage statement or mortgage papers/closing papers (for closing within last 60 days)
  • Signed and dated lease, and proof of last month’s payment (cancelled check or receipt)
  • Letter of Residence from Landlord in Lieu of Lease form (available on District 95 website) and proof of last month’s payment (cancelled check or receipt)
  • Letter of Residence to be Used When the Person Seeking to Enroll a Student is Living with a District Resident form (available on District 95 website)


CATEGORY II (TWO documents required)

Each document must have the current address:

  • Driver’s license
  • Vehicle registration
  • Voter registration
  • Most recent credit card bill
  • Current public aid card
  • Current homeowners/renters insurance policy and premium payment receipt
  • Most recent gas, electric, water bill (cell phone bills are not accepted)
  • Receipt for moving van rental


IMPORTANT: District 95 reserves the right to evaluate the evidence presented, and merely presenting the items listed does not guarantee admission.

WARNING: If a student is determined to be a non-resident of the District for whom tuition must be charged, the persons enrolling the student are liable for non-resident tuition from the date the student began attending a District school as a non-resident.

A person who knowingly enrolls or attempts to enroll in this School District on a tuition-free basis a student known by that person to be a nonresident of the District is guilty of a Class C misdemeanor, except in very limited situations as defined in State law (105 ILCS 5/10-20.12b(e).

A person who knowingly or willfully presents to the School District any false information regarding the residency of a student for the purpose of enabling that student to attend any school in the District without the payment of a nonresident tuition charge is guilty of a Class C misdemeanor (105 ILCS 5/10-20.12b(f).
