Attendance Exceptions

If you wish to request an attendance exception for the upcoming school year, please complete, print, and send this form to the Office of the Superintendent, District 95, 832 S. Rand Rd, Lake Zurich IL 60047. Approved attendance exceptions apply for one year only.
